But this tweet as is would just represent hearsay about what "she" said. Typically therapists do not have to contact authorities unless there is a credible threat (murder, serious crime etc.) about something going to happen.

There must be FACTS. People in therapy, like people anywhere, can say all kinds of things. Hearsay doesn't count.
