I watched that real time. Right after that a Trump staffer grabbed something of a similar size off his podium. Biden did look like he had an earpiece in though. And he clearly had the questions in advance.
He was looking up above the camera the whole night. At first I thought maybe there was a screen only he could see but maybe it was just him trying to concentrate on repeating talking points in his ear.
Who knows. All I know is they hide him for a week, load him with drugs like a doped race horse, ride him hard for 60-90 minutes and then put him away wet.
