They try to make the words distinct; that a fetish is a requirement to orgasm, and a kink is just something that you really enjoy.
Really, I don't think that's a distinction that doesn't need to be made. Fetishizing something doesn't inherently mean that you require it, and never has. I'm sure there are foot fetishists that have had sex just fine with double amputees and so on.
