The girl is holding the knife cutting the cake. The bandages...idk. They creep me out. With the tight angle of the camera I cant tell if there is anyone else around...especially other kids. It looks like there might be another adult arm in the pic, besides the picture taker. Just not enough context to really make a judgment here.
