For this photo to send the message that it looks to be sending she really has to be pregnant, and it really has to be Hunter's baby. Otherwise it's a crude joke/prank. I don't see this child making such a joke.
I won't say one way or the other since there is no information. I believe there has been more recent photos of her looking like her normal skinny self. The pregnant thing is kind of "Us Weekly" sounding at this stage imo.

Why? I read books like that at her age. If she went to a friend and bared her secrets and the friend suggested sending a message, maybe the friend might suggest holding that book. The girl need not actually be reading it to send that message, just pretending to be reading it is enough. But also, once the book is in her hands, with her grandpa's campaign and all, she might be curious, triple so if she's been molested by her uncle, eleventiple so if she knows her mom and grandpa knew and still it went on.
