Deep state and government bureaucrats. Anyone who has ever worked in any sort of government institution can attest to the absolutely deplorable condition of those systems.
Too often you run into people who say "yes absolutely, i'll get it done!" and they either forget to do it, do it half-assedly, or don't bother following through at all.
People who have any common sense or sense of pride in their job typically gtfo those settings, and what's left behind are cesspools of the lazy, inept, and apathetic. Compound that with the difficulty in getting anyone fired and you've got a recipe for disaster.
Because the spying was done by the Deep State and any prosecuting would have to be done by the Deep State too.
