There are states that only have a small percentage of the votes counted, and winners are already declared for some of the states. How reliable is it when it says Trump wins X state, and Biden wins Y state? Have they ever been wrong when a winner is declared before all the votes are counted for the state for the presidency?

They use statistical exit polling to determine winners before votes are counted.

I remember back in 2004 some states were called for Kerry that ended up Bush.

Yes but they are "no big deal" states where they'll correct it later because it doesn't typically make a difference mathematically since there are the other states have such big swing factors.
