If I had to guess? I would say small skirmishes like in Syria within major cities. Battles would be fought as "normies" try to just go about their lives. Fighters on both sides would slip in and out of "civilian" clothing just like urban warfare we've seen in Iraq. It would pop off between a group of antifags and someone like the proud boys maybe.
It won't be a large battle on an open field like in simpler times.
National guard and other military would be called in quickly and many of them splinter off to join other factions (Mainly pro-Trump anti-establishment IMO) as soon as they arrive, bringing with them gear and weapons. Escalating the conflict to heavy weapons. It would be very short and bloody. I am not sure how the media would cover it.
I would imagine the epicenters of it would be in DC, Atlanta, Chicago, Philly, LA, and Portland.
I expect local police to avoid them. The first few times they attempt to get involved they will be fired upon by both sides. War zones become "no-go zones" for cops. Many police will resign and join the fight or just go home to their families.
