I dunno about the US, but In Italy you only pay a minimum percentage of the cost for your medicines or health tratment if you go with the public option, the amount depends on your income, but the time to actually get treated is long, in the private you get very short queues, top tier treatment and the cost is not too expensive precisely because if it costed too much people would tell them fuck off I'll just go with the public option.
