By far and away the most current concrete and uncontested example of voter fraud is the banning/removal of poll watchers in places like Philadelphia.
This alone should invalidate those votes where this occurred as it is a direct violation of election law by one side.

Yushchenko was denied a voice by the media. He mysteriously got sick the month before the elections. The exit polls on election night showed him comfortably ahead, but there was a delay in counting. Then that night he was reported as having lost by a large margin of 2.5%. Overnight, there were large increases in turnouts but only in cities that favored his opponent. Reports found that data on the election commission was manipulated afterwards. There were reports on the ground of people delivering massive amounts of absentee ballots at multiple polling stations. As soon as Yushchenko lost, the state media was quick to declare the incubant the winner. Thousands of people immediately challenged the election and went out into the streets.
The parallels are simply uncanny.
I saw in a video where google search went up about election voter fraud and jail, prior to the election. I am not sure how to find it but it was in a mashup video here earlier. The women in AZ who failed to validate the systems, she resigned.
