Originally it just meant the account was actually owned by who it appeared to be (not some impersonator). It's manually added to the account. Except since twitter is part of these people's weird political religion, it was taken to be a seal of approval from the church. So then they started removing it from people they didn't like, but also adding it to random nobodies who were preaching the correct dogma.
