You don't give someone higher radiation burden really trying to pin down a sprain or hairline fracture. If they have pain and there was swelling and no notable large fracture on XR give a boot and say bye.

I have specialized knowledge of this subject, and this assertion is a red herring. CT scans are done for ankle injuries all the time. They are quite sensitive for subtle hairline fractures and avulsions and such, which may not be well seen on plain X-rays. They can also provide some limited information about effusions, ligamentous tears, and other soft tissue abnormalities, although MRI is more useful for the soft tissues.
Meh... in general, hairline fractures aren't anything you'd "fix" anyway. But I've heard plenty of GP's order CT's or MRI's of ankle sprains that really add 0 value to the actual diagnosis over what a basic weight bearing XRay would have shown.
On an ankle sprain, there is always some damage to ligaments and soft tissue. (Hence, the swelling.) Normal usually follows: XRay, no breaks / displaced breaks, boot, and follow up in a few weeks for further diagnostics if necessary. Something bad? Possible further imaging and / or surgery.
Not disputing that this may be a red herring, but for those 'pedes that don't have great insurance... definitely question further imaging as to what they are hoping to see / not see and why they feel they need to order it over a weight bearing XRay.
