2 in 250,000 is .008%, which, as I'm rereading this thing, might be the upward limit of acceptable. So 2 in 250k is ok, but any more than that is unacceptable.

1 vote too many is 1/249.999, that's exactly 1 vote too many, which is 1 vote over 0.0004% which is 0.0004000016000064%
So it's simply 0.0004% max allowed, no need to make it 0.0008%, that number is just wrong, unless the 250k is wrong like you said.
