I use a multitude of different needles and safety needles in my hospital.
This one for example is a syringe with a screw on needle that comes separately so that a different fatter needle or filtered needle can draw from a multi-use vial.
Hard to stick a needle in with the cap on... Imagine that?
Is not a cap, it's a safety flange that covers the needle to prevent the the person giving the shot getting stuck.
