All we need now are:

  1. competing facilities. Most cities outlaw opening facilities that aren't specifically requested by the city - healthcare is a carved out monopoly.

  2. the ability to refuse service. After all, if the hospitals all still have to cover non-paying homeless druggies and illegal immigrants, they are still going to have to over change those who do pay.

Don't forget removing government licensure. Licenses are one of many ways governments attempt to control people, doctors included, since they need licenses to sell their practice.
Can't speak up against certain hoaxy flus if your livelihood is at stake, you know.

Agree.

Licenses have a role, but they are too often used to carve monopolies through limits and stifle diversity by picking and choosing methods that are supposed to be left to the consumer.

I have no interest in stealing from the insurance holding paying customer behind me, so no, I won't do that.

It's all good, we just all start doing it. Collapse the broken system and start over.
