VP isn't POTUS's subordinate. There's a line of succession, and they run on the same ticket so politically the President is in control, but officially they are both chiefs of entirely separate offices.
The Vice President is elected, not appointed. He cannot be fired. He can only be impeached and removed.

Yeah, your last question answered it. The guy with the second highest number of votes used to become vice president, but as you can imagine, that might incentivize assassination or other issues.
