It was a virus that was put into an Iranian nuclear reactor system including but not limited to the control of the cooling system. Technically this could have led to a meltdown.

“ Stuxnet is a malicious computer worm, first uncovered in 2010, thought to have been in development since at least 2005. Stuxnet targets supervisory control and data acquisition (SCADA) systems and is believed to be responsible for causing substantial damage to the nuclear program of Iran.”

I work with SCADA in power distribution and ours isn't connected to the internet for reasons like this. I wonder how they got the virus into their system.
