They make the best guess and sometimes get it wrong. In addition, new strains/mutations are always popping up and not everyone gets vaccinated each year. They will never be able to eliminate all influenza deaths.
And somehow we got to the point that getting injected with who the fuck knows what every year by strangers who think they know better (but are openly guessing) is a good idea.
That would certainly explain why there are no flu cases anywhere in the world this last year
