His eyes look off because he has dementia. Mental problems totally change the way a person’s face looks, because of the eyes. He’s no longer processing information in the same way he did ten years ago; his brain is malfunctioning. The eyes are always the best way to tell this.

We have wayyyy too many portrayals of characters who are psychotic, insane, evil, literally demon-possessed, depressed, mentally retarded, and so on, who are played by cognitively normal actors. So those characters’ eyes look normal. (An exception to this was TDK’s Joker). But in real life, you can almost always tell by the eyes when somebody’s brain is abnormal.
