There are orders.

But we are all human beings.

If the order was given to nuke Canada I doubt anybody would just do it without hesitation.

An alternative viewpoint might be that we specifically chose people in the nuclear command chain that will obey orders without independent thought.

I heard a similar story about him getting drunk and giving out orders. It was said that the Joint Chiefs had an agreement to ignore them until he sobered up. Apparently, he wouldn't even remember doing it.
