A PhD does make you a doctor. Because a doctor has a doctorate, a published piece of work you can ask for from the institution. It is also the highest qualification in your field.
An MD is not a doctor as they don't hold a doctorate.

The reason why the title of "doctor" is used in English speaking countries comes from centuries ago when there was real overlap between medicine and science. And the concepts were not as separate as they are now.
