To piggyback off what u/MAGAtitle said, this is a low-effort Photoshop (but high effort meme!) of The Boss from Metal Gear Solid 3, in my opinion the best of the MGS games. There's a picture about midway down that page that shows her scar, from (as previously stated) a c section. She is also the visionary responsible for the events of every other MGS game, as 3 comes first chronologically. Her actions didn't really cause any of the shit that followed, but rather her profound impact on a few people.
Also, if you've never played the Metal Gear Solid games, I highly recommend them. There isn't another video game series which reflects on the nature of war and the nature of soldiers even a tenth so poignantly. There's a lot of insane shit too, for which writer Hideo Kojima has earned something of a reputation, and the cutscenes sometimes last quite awhile (Metal Gear Solid 4 holds the world record for the longest single cutscene at 27 minutes and the longest sequence of cutscenes uninterrupted by gameplay at 71 minutes). But MGS isn't so much a game as a story, and one with powerful and important messages, so those things are forgivable.
