Fiona's jaw is wide with big jowls + muscle, but still V-shaped, and her forehead is tall, but rounded and narrows as it goes up (see her 3/4profilepics).
Look at the difference between her forehead/skull and the man’s here and here; this part of the body can’t be changed by surgery or exogenous hormones. MTFs can get their hairline pulled down, but the skull shape doesn't change, and Fiona has a high, natural hairline. Notice how the man’s jaw is small, but the ramus is still straight like a man’s should be, which is unlike Fiona’s.
Her back is also curvy, which is a female trait. Notice the difference between her and the man's backs here.
Fiona's upper torso also lacks mass and she looks like she has normal female shoulders and q-angle (how the thigh slants inwards).
