If you in your basement & away about 20 miles from air/surface detonated epicenter, you have clean water & potassium iodide ki to protect your thyroid gland you will be ok depending on the altitude & magnitude you can probably start safely getting outside in a week or 10 days. You have to be careful not to ingest dust/ash as a residual radioactive material in a fallout. Surface or air burst detonation there are two types of fallout variety 1. small amount of carcinogenic material with a long half-life 2. large quantity of radioactive dust and sand with a short half-life. If surface burst you get #2 if air burst & fireball does not touch surface you get #1 in case of fireball touching surface you get a mix. If you inhale it it's bad, if you got it on your skin you can quickly scrub it off otherwise it is also bad. In best case scenario you want to safely move away from ground zero for 300-500 miles away, wear respirator & no exposed skin.
