Usually you'd get a lawyer to draft up power of attorney. I'm not sure what kind of services the VA has, but you have other options for medical / general power of attorney. You could also file through the courts that your father lacks capacity and have them grant it.

its a free service from VA they will send a social worker.... power of attorney transfer. hes clearly in decline. its more pre-emptive at this point. hes still cognizant till about 6-8 pm. but he is declining and its frustrating to wait when they are volunteering to drive out and give him a shot of poison but cant do standard shit.

go online download a form & do yourself & get a Notary to stamp it with two witness's as you two sign it.

thanks. i'll be doing this if the social worker is a no show again. a household of people, him included, wanting to get it done before its too late.
