Think about the actual steal and then the cover up and how many people, agencies and organisations that were involved in this, not only in the US but also internationally where the mantra 'there is no proof whatsoever of any fraud, it is just another crazy right wing conspiracy theory" was dominating. It would not have run this smoothly as it did for them if this had been the first time, they must have been doing this many times before, only this time they had to go so big it was no way to hide it.
It doesn't have to be a big crazy conspiracy; its pretty simple to figure out to harvest/create ballots when you need them.
There really only needs to be a small number of people to pull this off across multiple states. A couple poll workers per country at a single polling station is all it takes.
