These people say they do this because they they want to feel "comfortable in their own skin". Social comfort is a function of acceptance, fitting in etc. I refuse to believe that this guy (or anyone) feels more "comfortable" looking like a revolting freak. They're going out of their way to look like freaks. It's a sexual fetish. They get off on it.

Having everyone you walk past be repulsed by your appearance is not a path to "feeling comfortable in your own skin". If he wanted to feel comfortable (i.e. not be seen as a freak) he'd get a haircut, wipe the makeup off of his face and dress normally.

Society needs to stop humoring perverts who indulge their fetishes in public and force everyone around them to participate in these sexual fantasies without their consent. For god's sake, this guy even interacts with children while he's acting out his fetishes. That's unforgivable.
