I have advocated for years that all doctors that perform procedures should display a spice rack they have built with their own hands along with a certificate of grading by any 7th grade shop teacher for fit, finish, and overall workmanship. If you have seen as many misplaced screws and unused drill holes in bones of all sorts and the utter confusion as to why a guide wire isn't following an artery displayed by practicing physicians as I have, this would be a no brainier.

After the guide wire has been inserted and verified to have entered the structure the needle was used to access, the needle is pulled out and removed over the guidewire and discarded. A hollow tube, of many varying types used to do different things, is then slipped over the guide wire and using the guide wire as a path, inserted into the cavity the guide wire in now in. The guide wire can then be removed leaving the hollow tube. You now have access to the cavity to administer medicine, take pressure readings, inflate a balloon to smash plaque in an occluded artery, inject material to occlude blood flow to a tumor, insert a blood clot filter, inject dye to see it on x-ray, lots of stuff. Of course things can go wrong due to operator error but sometimes shit just happens. It's never a viable excuse but experience and caution (not being a cocky shithead, happens more than you know) reduce the risk a lot.
