How do you fuck up so bad that you decapitate a new born? I mean I know they're babies and they're pretty fragile but that would still require a decent amount of force. Like intentional force.
I remember reading about it at the time. Basically, she (the doc) was tired at the end of her shift and wanted to go home, but her patient had only been in labour a little while and was only 4cm dilated. Instead of waiting for the woman's labour to progress enough to deliver the baby safely, she decided to use forceps to pull the baby out. Then she ripped its head off.
