Maybe he is telling the truth - he did not receive covid vaccines, and what he received was "sugar water" because he would be dead if he received the vaccine.
Biden isn't really in control of what he says... there's a disconnect between his conscious mind and his mouth, and he does sometimes (accidentally) tell the truth. His dementia is an unreliable truth serum.

So after 30 years of Biden lying for political gain, he is suddenly a whistle blower for the truth. Yeah that seems like the most likely theory...

This was my first thought as well. He has a habit of accidently admitting things like this.
He also has a habit of lying about the successes of the Trump administration.
