It is my understanding that an extreme hacking from an irritant can cause vascular damage if there is an underlying weakness in the blood vessel. I heard one case where a man with a sore throat coughed so hard he had a vessel start to rupture in his middle ear. He realized something was wrong when he could hear the blood rushing into his head.
