A lot of them also have empathy disorders. A lot of people go on about a lack of empathy but fail to appreciate that people also have severe empathy-apathy disorders.
You can compare it to eating disorders. Some people eat too much, some people not enough. Some people eat the wrong thing. Some people go so far as to really eat the wrong thing such as inedible things (shoe laces or marbles for example). Some people eat for the sake of eating then throw up to eat more.
The same range of disorders exists with empathy or sympathy and these are all under heavy attack from fake news, fake charities, etc. Charlatans always target common psychological flaws. As eating disorders go a lack of hunger is quite rare. I believe empathy disorders are the same.
A simple example of an empathy disorder is the baby is crying disorder. You can't reason with the baby you just have to give it what makes it stop. The disorder that if it makes them feel bad it's wrong.

I would buy them another one but I don't want to be sent to prison as a paedophile or for rumours to allow people to mistreat me so I tell the little shit to get lost or I'll really give them something to cry about.

It's like being colour blind. Some people see more than one channel of meaning. They see extra channels of meaning but they are pessimistic, paranoid and often wrong.
They are unable to see the funny side of things. This is a very serious disorder which can result in total psychological breakdown. It's why two normal adults can talk about dark things that would normally make you feel bad and also laugh but people on the left go into a screaming fit about that they feel bad because they have no humour to compensate.
There's an element of empathy that's contagious. They have errors in this where they do not learn the feeling of humour properly from others but dark feelings circulate freely among them. Bad empathy goes around. They mirror the improper emotional reactions of others.

I can speak from experience that the OP is correct.
A couple of years before Trump became president, I befriended someone who I mistakenly believed was a kind person because of how much she emphasized on empathy. During the course of our friendship I grew to realize she was a covert narcissist whose little world was revolved around herself. She had no empathy for anyone unless their problem related to her own self-identity. Her values were quick to disappear when conflict arose and then she would get petty grudges against people. She had no problem using your time and your effort for her emotional validation, and then was quick to forget you existed when she no longer needed anything.
She is also leftist who increasingly grew to push nonsense about trans, 3rd gender, otherkin type of crap and feminist victimhood. She exists in the paradox of viewing herself as a female victim while at the same time believing she has no gender.
I was very miserable from the experience of being her "friend", but looking back I'm kind of glad I learned from this experience, because it prepared me to recognize the phoniness in all of the leftists during the Trump years.
