Oh man. It’s really sick.

They basically skin you (either arm or torso) and roll it up into a flesh tube.

Then they create a pathway for the urethra so you can urinate through it.

Sometimes they install hydraulics so you can manually inflate it to get an “erection”.

They can close the hole off and make a fake penis and fake scrotum. Bottom surgery may (but doesn't always) include removing the uterus and/or ovaries.

Obviously if the ovaries are removed the person is permanently sterile. It's possible to freeze some eggs before removing the ovaries, but a surrogate mother would be needed to carry the baby.
