Laws vary state to state. If your state has at-will employment then they can fire you because they think your shirt is ugly.

This sounds innocent enough but I don't doubt that if your name goes on the survey then it will be used to track who gets it and who doesn't.

That’s exactly the part I’m worried about. At the bottom it says the part about your response being “added to your health record in [our workplace health tracking software, same place they keep records of our flu shot]”.

I'm assuming it's because OP is a healthcare worker and somewhere in their contract they gave up their HIPPA protections.

Even if a heathcare worker, I would be having my medical records keto at a place where my employer dies not have at will access to them. Not their business.
