There’s nothing here saying you’re required to take it, it looks like it’s just a standard survey so they can get an idea of how many staff are vaccinated.

Currently there’s still not enough vaccine to go around, and they’re distributing it via a lottery system in many circumstances, so they probably won’t require it yet, if they do at all.

Since you’re in the healthcare industry there might be different rules about immunizations, but even after getting the Covid vaccine you’re supposed to act as if you never got it, so I’m not sure what good the vaccine will do in the long run.
