it was found by standard fraud analysis... they were super bad at cheating, and would just go down the answer sheets the kids filled out, and for example, they'd fix all answers where kids got any of 6-10 wrong. this resulted in literally 100% of students getting that block correct, and erasure marks are also detectable and tracked, making it painfully obvious.
NCLB did not effect how teachers got paid. It forced some bad teachers to have to go through extra training, but it did nothing in terms of fixing the payment system. I did a whole project on this back in college.
But you are correct that standardized tests are an inadequate measure of which teachers are effective.
