So none of them were in ketosis?

Easily could have been, alcohol doesn't knock you out of ketosis. Your body just works to get rid of it first so it stalls weight loss. It's said 200 years ago average intake of sugar per year was around 2 lbs. That averages around 3 grams per day, well below ketosis levels. Now carbs from breads, fruit, and beer that's another matter. However I'd imagine there were plenty of people that survived mainly with meat from hunting, they would most certainly spend large chunks of time in ketosis.
