This is a low quality version of it. Very low quality. There's a much better PDF going around that is 100% correct. If someone tells me how to share a PDF effectively on this website, I'll link it.

It is NOT foldable! It is printed on both sides. It appears to be 40-60lb card stock imo. DDG "covid vaccine card" and look at images.

There are two revisions of this card:

My feeling is that is is strongly likely that most private individuals "getting" this vaccine would have the 09/03/20 revision card, as the earlier ones were either never passed out or only selectively so (image search indicates some of them were used for patients as they are shown filled out).

I believe an edit of the 08/17/20 PDF would correct this issue, provided there are no other changes (Edit: the text at the bottom of the back page is also slightly different between versions with different spacing due to the reduced spacing in the top portion of the back page).

There are definitely differences - the one I linked is from the Wyoming Department of Health, however, not the CDC.

It is also the same as the image provided by OP.

Edit: Here's one from a PA dept of health website that appears the same, with the "or" on the 1st line instead of the second.

Ediit: Here's another one that appears to be printed out and filled with both vaccine shots that has the "or" on the 1st line.

Edit 3: Here's another what appears to be an official card with the "or" on the 1st line from a news article in Kansas.

 

What I'm seeing from image searches is that most of them have the "or" on the 2nd line, but some do have it on the first line as well.

I think you may be on to something here and users should beware of this, but I'm not entirely sure of the significance because the printed cards appear in both fashions. I believe the PDF and OP version is an earlier revision of the card, dated 08/17/20. I am attempting to verify that now and will get back to you in a day or two hopefully.

Edit 6: There's an 09/03/20 version on the card that you see in this picture.

 

Interestingly, the top line "or" has been used on this article about people using a fake negative Covid-19 printout in order to travel. super meta.

I was the "good kid," who wasn't actually "good," but just managed to have the same last name as my older brother, who was the golden student at my high school years earlier and thus associated with him... so with that, along with getting decent grades/being all honors, I got a lot of free passes and was just seen in a better light than I should have been. Because of that, my friends and I always got out of trouble for skipping class, going to taco bell for lunch, etc. All they had to do was basically say they were with me, and all was good. It was pretty nice lol. I was kind of like the (male version of the) chick/main character (lindsey?) in Freaks and Geeks and my friends were the freaks. Except I was a freak too that was just seen as not being one because of my brother, if that makes sense.

Man I miss high school. I mean, I don't really... but I do.

Yeah... it was a weird experience that I definitely view through rose-colored lenses, kind of glorified by hindsight. Like... there's probably more I don't miss than what I do miss, but sometimes nostalgia takes hold and leads to me missing those years. I think it's more just being that age, etc., that I miss, more so than actual high school/going to school that I miss. Not to mention the whole "man, if I knew than what I know now," kind of thoughts. In other words... I'd love to go back to my high school years (and friends, experiences, etc), but maybe not necessarily high school and so some things the same, and other things totally different.

When I visit back home, I almost always inevitably have a "Glory Days" (Bruce Springsteen... forgive the lame reference) kind of experience (whether it be visiting old friends, or running into old classmates I haven't seen in nearly 15 years) and I think it's those experiences that make me nostalgic and miss it all.

I was in a sense (as I was on the baseball team, but far from the best player), but I guess I'm more Bruce in this song (only instead of being a famous and douchy musician with horrible politics, I'm a not-famous, not-douchy musician with the right politics lol). Tape over the yellow on your printer before printing. This has no yellow, so it will not impact the print job. All modern printers embed a matrix of yellow dots throughout a printed image. This tracks things such as date / time of printing, the make / model of the printer, and much more.

Point being, at first, they wont give a shit. After some major company decides they had an "outbreak", thanks Dustin Hoffman, the government will mandate verification. I don’t know much about paper but it’s closer to social security card type card. Maybe modern selective service cards (I’m 32). No creases or folds in it.

looking at this image, it appears to be flipped to me, so if you were to flip it with the left edge moving forward and around, the back would be displayed in a readable alignment.

edit: If you watch the video on this news article and follow the card they show at 0:25 with the fold crease down the middle, you can see it again at 0:33 and that shows how the back image is flipped vs the front image.

Mine had a sticker that said "J&J Dose #1" and the lot #, date received, and location. The patient name and information at the top was blank and they expected you to fill it out. These cards were just stuck in the folder they handed you with other information. You could have just left after they handed you the folder. Nothing proves you actually received the shot.
