There have been videos of multiple angles spliced and timed together to show that the microphone was on a long boom behind his right arm. That, combined with the lens, angle, and video compression on a fuzzy object resulted in a jarring video that looks suspicious at first glance.
Let's look at it from the other direction. What do you think would be easier to get a senile old man to do?
Perform a highly choreographed and scripted act in front of a green screen that then has to be edited and passed on as authentic.
Have a dottering old man stumble out to a handful of friendly journalists and fail to answer two questions before getting on a helicopter.
Then there is the fact that if it was fake, it was done so intentionally. Having Biden's hand go both in front and behind the dead cat mic means that there would have been extra editing complexity because you then have to keyframe both the mic and Biden instead of just Biden.
