Go online and get a copy of a card and fill it out. Or tell them you have a medical condition that prevents you from getting one. I think one thing is allergic reaction to penicillin, anaphylaxis . Unless you don't care and then do what I would do and say "No"
But this way its documented. That way you can say they are forcing you to do this even though its contraindicated. Not sure if it will work, but just a suggestion.

had not heard about that and i am allergic to penicillin. ill have to look this up.
Ask your MD for your records and if you wrote it on your history its your condition.
