At some point they have to cut off his meds enough such that "DOCTOR Jill" doesn't make a fuss when they toss him aside. So long as Joe is somewhat functional his wife gets to shine as his caregiver. But if Joe goes full "who are you" mode then even Jill won't be able to fight for her plum position.
