Only thing I can think of which would be a problem would be using the CDC logo or any markings which state it's an "official government form".
Strip those away (people can add their own) and they should be gtg.

remember to strip all EXIF data from both images and pdf files before sharing.
for linux, there's a simple program called exiftool in most repos that will remove EXIF from both PDFs and photos.
In Windows, maybe someone else can say?, I don't know.
