From what I understand when little kids act out sexually (not innocent playing Dr.) it usually means they have been molested.

Have you ever met an adult female with a really high pitched child like voice? That's also an indicator of past sexual abuse interestingly. Not always of course.
I didn't realize that was an indicator, but can confirm, the two women I've known who talk like that I eventually learned had been abused.
