This distinction is fairly empty. Every democracy has a structure. "Republic" is simply the structure of US democracy.

A constitutional republic is still ruled by a majority. It cannot be anything else. The only alternative is rule by minority.

The constitution requires 38 states out of 50 to ratify a change. That's rule by majority, it's just a majority structured a certain way.
