Throwing this out here as a wild suggestion, but what if the vaccine purposefully targets that specific mitochondrial disorder (but doesn't effect anyone else) in order to target a specific part of the population, or even just to test that the genome targeting works without raising any suspicions that it was targeted, e.g. by claiming: "looks like it was the fault of the rare disorder having an adverse reaction", averting suspicion that it was known from the beginning that it would have that reaction.
You could target any groups that share part of a genome and have a vaccine that only adversely reacts to them, for example.
