There will be two judgements, the one that Revelations refer to is the general judgement at the end of the world. Catholics believe that at the time of death there is a particular judgement that determines the soul's place for eternity, heaven, hell or purgatory, (a time of suffering for the temporal punishment due to sin that was not expiated in time or on earth. Spent there until Jesus determines otherwise.)
revelation also references death and hell being cast into the lake of fire: "And the sea gave up the dead which were in it; and death and hell delivered up the dead which were in them: and they were judged every man according to their works. And death and hell were cast into the lake of fire. This is the second death. And whosoever was not found written in the book of life was cast into the lake of fire." Rev. 20:13-15
