There are state licensing requirements, including exams. Just graduating medical school doesn't mean you practice medicine.
But yes, we have that problem in medicine. Most of the unqualified couldn't get into a residency program, so they aren't specialists.
As a physician - licensing exams are so easy it's a joke. I do agree that some people can't get into residency but those are usually people who bomb interviews. Plenty of charming idiots get into and pass residency and become doctors.
