If I had to guess, I would say the cutting of the cane with machetes would have resulted in frequent cuts which in those days could have led to infection and death.
Thanks. I also did a search, and apparently, in recent years, there is a high death rate among sugar cane workers due to kidney disease. They're not sure why, but it may have to do with the chemicals being used, and/or the burning of waste materials.

I would imagine the areas for sugarcane were also much more rife with tropical diseases
