I dunno. If you look at the tip of a vanishpoint syringe, there is a compacted spring in the tip. It's black, and visible. After it pops the needle back, the tip looks white (clear) as the spring expands. In that video, I don't see a black area in the tip. It doesn't change from beginning to end that I can see.

It's real.. but the needle retraction is not supposed to be done while still in the patient.

Every vanishpoint syringe I've seen used has to be retracted by pressing a mechanism. Generally makes an audible click.
