Does anyone recall the mad cow disease scare from the 90s, that was epicentered in Europe?

IIRC, the epidemic started because ranchers were feeding their cows other dead cow carcasses. I wonder now though, weren't they also vaccinating their cows and giving them antibiotics as well. Is this a trifecta of doom?
